Covid Release Form:
This form must be submitted 24 hours before your scheduled appointment. Click on link to the right.
Day of your appointment:
You must arrive 10 minutes before your scheduled appointment to allow enough time for parking, restroom use and etc. If your temperature reads over 100F you will be rescheduled.
During Service:
Blankets will no longer be available but the bed will be heated to keep you warm. Please do dress comfortably.
After Service:
A recommended at home skincare routine will compliment your treatment and transform the health of your skin with plant-based products which will be available for purchase. Please bring a reusable bag if you plan on shopping for skincare goodies.
Sanitation Standards:
Stations, treatment beds, machines, tools, and products will be fully cleaned, sanitized, and disinfected between each client with hospital-grade disinfectant. Trained and certified for infection control with Rejuvenate and Barbicide, as well as Barbicide Covid-19 education.
Booking Policy
Your reserved time is for you! Clean Beauty Quyen is currently accepting fully vaccinated clients and you will require so show proof. I truly value your business and ask that you respect the policies implemented. Should the need to cancel or reschedule arise, please notify me 72 hours prior. Appointments canceled less than 24 hours are subjected to a cancellation fee amounting to full cost of scheduled service. Clients who miss their appointments with no prior notification will be charged in full of scheduled service. Failure to submit the Covid-19 Health Questionnaire 24 hours before your booked appointment you will be subjected to a charge of 50% of your booked service.
